This information is required of ALL APPLICANTS. Submission of an item does not guarantee it will be approved. Item approval is at the sole discretion of the event coordinator. You will be notified if any items are not approved.

Food Vendors: You may submit your menu in lieu of this form if prices are included for each item. If using this form, you must itemize your products and include prices. For example, if you sell both cheese fries and regular fries, do not simply put “fries” even if they are the same price.

All Other Applicants: You do not need to itemize your products by size, color, flavor, etc. For example, you should list “cookies” not “chocolate chip, sugar, peanut butter” or “hats” not “red, blue, & green hats.”
